A structured observation form for managers and continuous improvement leaders conducting a Gemba walk — going to the actual place where work happens to observe, ask questions, and identify improvement opportunities. Covers the seven key observation areas: safety conditions, workflow and process, quality at the source, visual management, standard work adherence, equipment condition, and team engagement.

Gemba is a Japanese term meaning "the real place." Unlike desk-based reporting, a Gemba walk puts leaders face-to-face with the work, the workers, and the waste. The goal is not to audit or police — it's to understand current conditions, show respect for the people doing the work, and identify opportunities for improvement through direct observation and conversation.
