Healthcare-associated infections (HAIs) are among the most preventable causes of patient harm. This audit checklist supports infection prevention and control (IPC) teams in systematically verifying compliance with hand hygiene protocols, PPE use, environmental cleaning standards, sharps disposal, linen handling, waste segregation, isolation precautions, and equipment decontamination across clinical areas.
IPC nurses or designated auditors should complete this form during unannounced spot audits on each ward or department. All Unsatisfactory findings must be reviewed with the ward manager within 24 hours and a corrective action plan documented. Aggregate audit data should be reported to the IPC committee monthly and used to target improvement initiatives.
