This version covers the three critical phases — Sign In (pre-procedure verification), Time Out (immediately before incision), and Sign Out (before patient leaves the OR) — ensuring the surgical team verifies patient ID, procedure, site marking, allergies, equipment, and post-op plans.
Consistent use satisfies Joint Commission NPSG.01.01.01 (patient identification) and UP.01.01.01–03 (Universal Protocol) requirements and is a core CMS Condition of Participation.
