Emergency showers and eyewash stations must deliver tepid water at adequate flow the instant they are needed. This log provides a weekly inspection format per ANSI Z358.1-2014 requirements — activation test, flow adequacy, tepid water temperature (60–100°F), drainage, signage, and 10-second travel path clearance.
Weekly activation flushing also prevents stagnant water and biofilm build-up. Documented inspections satisfy OSHA 29 CFR 1910.151(c) and support ANSI Z358.1 annual certification by demonstrating consistent maintenance.
