OSHA JHA form for step-by-step task hazard analysis with identification, risk ranking, and control measures.
A Job Hazard Analysis breaks a task into steps, identifies the hazards at each step, and determines the controls needed to keep workers safe. This JHA form follows the OSHA methodology: task description, step sequence, hazard identification, risk ranking, and control measures (elimination, substitution, engineering, admin, PPE).
Documented JHAs satisfy OSHA's general duty clause and demonstrate that hazards were systematically assessed before work began. They are used for routine tasks and as part of permit-to-work systems.
