A workplace hazard assessment form for safety officers evaluating personal protective equipment needs per OSHA 29 CFR 1910.132. Walks through hazard identification for each body region — head, eyes and face, hearing, respiratory, hands, body, and feet — and documents the PPE selected to address each identified hazard.
OSHA requires employers to perform a hazard assessment of the workplace to determine if hazards are present that necessitate PPE use. The assessment must be documented in writing with the certification date, workplace evaluated, and person certifying. This form satisfies that regulatory requirement.
