OSHA Form 300 log format tracking work-related injuries and illnesses for annual recordkeeping and posting.
OSHA 29 CFR 1904 requires most employers with 11+ employees to maintain a log of work-related injuries and illnesses. This simplified log format captures the Form 300 data: case number, employee name, job title, injury date, location, description, classification (death, days away, restricted, other recordable), and lost/restricted days.
The 300A Summary must be posted February 1–April 30 each year. Completed logs support OSHA recordkeeping compliance and workers' compensation management.
