A semi-annual roof condition inspection form for facility managers and roofing contractors assessing flat and low-slope commercial roofing systems. Covers membrane or surface condition, flashing integrity at edges and penetrations, drainage system function, parapet wall and coping condition, skylight seals, rooftop equipment base waterproofing, and any signs of water ingress from below.

Roof leaks are the most common cause of building interior water damage and they're almost always preventable through regular inspection. Most roof failures start as small, repairable defects — a split in a membrane seam, a cracked flashing, a blocked drain — that go unnoticed until water appears inside the building. Semi-annual inspections catch these issues while repair costs are minimal.
