OSHA 29 CFR 1910.178(l) requires formal training and evaluation before operators can drive powered industrial trucks. This checklist tracks the OSHA-required programme: truck-related classroom topics, workplace-related topics, hands-on practical training, pre-shift inspection, and performance evaluation by a qualified trainer.

Documented training records are the first thing OSHA checks during forklift-related citations. Refresher training is required every 3 years or after an incident/near-miss.
