Confined space entry is one of the deadliest workplace activities — atmospheric hazards, engulfment, and entrapment kill workers every year. This entry permit checklist covers the complete OSHA 29 CFR 1910.146 process: space identification, hazard assessment, atmospheric testing, ventilation, isolation, PPE, communication, rescue plan, and entry/exit logging with entrant, attendant, and entry supervisor sign-offs.
