An equipment issuance and return form for HR and IT departments documenting every item provided to an employee — laptop, mobile phone, keys, access cards, PPE, tools, uniforms, and company vehicles. Records the item description, serial or asset number, condition at issue, employee acknowledgement, and eventually the return condition at termination or transfer.

Equipment that isn't tracked at issuance becomes equipment that isn't recovered at departure. Missing laptops with company data, unreturned keys, and lost tools add up to significant cost and security risk. This form creates the signed record that ensures everything issued is eventually returned — and in what condition.
